Report an incident

Not sure what to do if something goes wrong on site? Here’s a step-by-step guide to reporting incidents and when to escalate it to WorkSafe Victoria.

If an incident occurs, your first priority is safety. Where it is safe to do so, take immediate steps to prevent further harm or damage:

  • Assist any injured person(s)
  • Make the area safe
  • Isolate or switch off any equipment involved
  • Provide first aid
  • Call emergency services (dial 000) if required
  • Secure the area and restrict access

These steps should be taken before reporting the incident.

Incident reporting

Once the situation is under control, report the incident as soon as possible to your:

  • Immediate supervisor
  • Henley Homes site supervisor
  • Construction manager or a member of Henley Homes’ Health, Safety & Environment (HSE) Team.

When reporting, include:

  • Names and details of people involved
  • A description of the incident or near miss
  • Nature and extent of any injuries
  • Photos of the incident site (if safe to take them)

Notifiable incidents

Some incidents must be reported to state or territory regulators (e.g., SafeWork NSW, WorkSafe VIC, WHSQ). These include, but are not limited to:

  • Serious injuries requiring hospital admission
  • Head injuries, fractures, deep lacerations
  • Electric shocks, fires, explosions, chemical spills
  • Collapses or failures involving structures, excavations or equipment
  • Dangerous occurrences involving plant or machinery

If you're unsure whether an incident is notifiable, stop work immediately and contact your supervisor or a member of the Henley Homes HSE Team for guidance.
